Office Policies

Cancellation Office policy

Please allow 24 hours to cancel your appointment and 48 hours for any laser treatment appointments. “No show” appointments will be automatically assessed a cancellation fee.

Payment is due at the time services are rendered. Payments may consist of insurance co-payment or full payment for uninsured patients.

We accept most PPO insurance plans, please check with your insurance provider for coverage details. Once your insurance has processed a claim for your visit, you will receive a bill via email or regular mail. There will be a maximum of 3 bills sent for accounts that may have an outstanding balance. After that your account will be considered delinquent and may be sent to collections. For your convenience you can opt-in to receive your bill electronically with an option to pay online through our secure Kareo Billing-Stripe payment portal.

Our office accepts Visa, Mastercard and American Express as well as cash and checks. Please make checks payable to Alan Rosenbach, MD.

Per HIPPA guidelines, medical records shall be requested in writing to ensure your privacy. All patients may request a one-time free of charge copy of medical records. Additional copies may be requested for $35 thereafter.

Returns policy

We will gladly accept any open or unopened products for any reason up to 60 days after purchase and we will refund your original form of payment. Please call the office if you have any further questions.
